New Urgent Community Response service will bring health care closer to home across Notts




A new community response service providing urgent care has been launched.

Nottinghamshire Healthcare has announced its Urgent Community Response (UCR) service across adult physical health community services.

It will provide a vital role in the delivery of urgent care to people living in Nottinghamshire including Bassetlaw, mid Nottinghamshire and south Nottinghamshire.

The delivery of the service is in response to NHS England’s national mandate to accelerate the treatment of urgent care needs closer to home and prevent avoidable hospital admissions.

It sets out that all Integrated Care Systems (ICS) must ensure UCR services are available to all people within their homes or usual place of residence including care homes by April this year.

The county-wide UCR service provides a two-hour response to support a person who is at risk of admission or re-admission to hospital due to a ‘crisis’ and they are likely to attend hospital within the next two to 24 hours.

The focus of the UCR is to deliver a clinical intervention to prevent further deterioration and to keep them safe at home.

Becky Sutton, director of community health service at the trust, said “We’re really excited to be able to go live with our new and expanded urgent care response service. The service will make a huge difference to patients meaning we can assess and put additional services and support in to keep people at home.”

The service is accessible to all health and social care practitioners through a single point of access approach and is available seven days a week, 24 hours a day.

Gemma Whysall, ageing well programme director at NHS Nottingham and Nottinghamshire Clinical Commissioning Group (CCG), said: “Nottinghamshire Healthcare has worked with system partners to deliver the new expanded UCR offer which is a key part of our system-wide ageing well programme. The UCR service will support the system to keep people safe and well at home across Nottinghamshire.”

Nottinghamshire Healthcare’s community service division continues to work with system partners to play a key role in delivering urgent care for the Nottinghamshire population, bringing urgent care closer to home.
